Schedule Details:
- 1 full-time position (40 hours / week)
- Will be based at our Eau Claire – West location serving multiple Dove Healthcare locations
Required Qualifications
- Professional health care experience required (social worker, nurse, CMA, CNA, pharmacy tech, PTA, COTA, etc.)
- Ability to multitask, juggle multiple priorities and work in a fast-paced environment
- Proficiency in relevant software tools / applications such as Microsoft.
- Experience with EPIC/Care Anywhere, Point Click Care (CRM) is helpful.
- Detailed oriented with strong organizational and time management skills
- Strong problem-solving abilities
- Proficiency in written and verbal communications
Working as an admissions coordinator, you will be responsible for:
- Coordinating admissions with hospital discharge planners and other referral sources
- Assessing, coordinating and ensuring a smooth admission transition
- Confirms medical and financial information needed to ensure a smooth admission transition
- Develops and maintains excellent networking skills and relationship building with referral sources
- Ensuring patient continuity of care
