Position Title: Community Support Coach


Department: Adult Day Services


Reports to: Program Manager


Status: Full-time, non-exempt


Hours of Operation: Monday - Friday 8AM - 4PM


Summary

This position teaches skills and behaviors to maximize independence in activities of daily living for a small caseload of individuals who have severe developmental disabilities and other special needs in a variety of community settings.

Essential Functions:

Direct Service

  • Assists persons served to increase their independence and to exercise their rights by teaching skills in various activities of daily living (self-care, care of the household, communication, leisure, etc.) according to formal plans and as natural teaching opportunities arise, using informal teaching techniques/procedures.
  • Assists persons served to reduce maladaptive behaviors and to learn appropriate replacement behaviors by implementing approved behavior management procedures, according to formal plans and as natural teaching opportunities arise.
  • Assists in maintaining the health, hygiene and grooming of persons served by teaching new skills, implementing first aid as needed, health care and hygiene techniques according to instructions or plans and, if properly certified, by administering medications according to stated policies and procedures.
  • Provides active treatment by continually engaging persons served in age-appropriate, functional activities, planning and implementing community access activities, and implementing programs to achieve ISP outcomes.
  • Transports clients served as needed to community activities, following Faison vehicle training and using Faison-provided vehicles.

Administrative Activities

  • Documents care/assistance provided, behavior management and teaching techniques implemented and compliance with various health and active treatment standards through accurate and timely completion of progress notes, checklists, records, schedules, and billing information if applicable.
  • Completes quarterly reports for all clients served, as assigned by supervisor.
  • Maintains compliance with applicable federal and state regulations, and Faison policies and procedures. If assigned, serves as the Qualified Intellectual Disabilities Professional and liaison for all matters affecting persons on caseload during program evaluation and regulatory reviews.
  • Accesses resources appropriately to ensure the health and safety of individuals served.
  • Participates in the planning process by attending staff and team meetings and ISP sessions.
  • Assists with training new staff members as directed by supervisor.
  • Attends assigned trainings and implements procedures as outlined in trainings or in training materials; completes all assignments related to assigned trainings.
  • Serves as the Staff in Charge, as needed and delegated by supervisor, Director or VP. Responsibilities of the staff in charge include: ensure notification of relevant parties if a significant incidence occurs; ensure appropriate caseloads are maintained; orienting substitutes; ensure implementation of daily schedules; ensure appropriate documentation is maintained; ensure medications for all individuals are administered at appropriate times and maintaining knowledge of the whereabouts of all clients and staff when out in the community.

Education and Experience

  • High school degree or equivalent required.
  • Bachelor’s degree in a related field preferred.
  • Previous experience working with individuals with disabilities preferred.
  • Certification or able to be certified in CPR/First Aid.
  • Able to operate a motor vehicle with proof of valid driver’s license and an ability to qualify for as a driver under Faison insurance guidelines.

Physical Requirements

  • Must be able to lift up to 50 pounds while keeping back straight.
  • Bend all joints fluently.
  • Move in and out of and maintain a kneeling position for up to five minutes.
  • Reach in all directions.
  • Grasp using both hands.
  • Move quickly in both confined and open spaces to avoid safety issues.
  • Run in order to retrieve an individual who is moving quickly away from staff.
  • Maintain balance while lowering to a half squat position.
  • Ability hear, see, and speak.
